Botox Consent Form

This consent provides me in an educational way and at the same time informs me about the Botox treatment, so that you can make the decision to carry out the procedure or not, knowing the risks and benefits of the treatment that will be placed in an aesthetic way.

I have asked to Finer Beauty to try to improve the fine lines of expression on my face with the botox treatment. This name refers to botulinum toxin. These injections have been used for several decades in children and adults for muscle enhancement. This toxin has also been used to correct double vision due to muscle imbalance. Botulinum toxin injection `produces relaxation of the frowning muscles, and the same to treat facial wrinkles. Although the results are dramatic, it is reported that the practice of medicine is not an exact science and does not guarantee 100% expectation.

The injection of the toxin will enter the muscle with a small needle, the effects of botox can begin to take effect 48 and 72 hours after its application. The patient notices that he cannot contract the muscle no matter how hard he tries, where this effect becomes total or complete between 7 or 10 days after its application. I understand that it can last 6-8 months in the area of ​​application.

The side effects that may be experienced or more frequent are: difficulty swallowing, muscle weakness, rhinitis, cough, throat irritation, headache, loss of skin sensitivity, malaise or fever.

It is important that you keep the following in mind and that YOU WOULD NOT BE A CANDIDATE FOR A BOTOX APPLICATION:

-Being pregnant or lactating.
-Hypertensive patient without being controlled.
-Uncontrolled diabetic patient.
-Next to take a trip and by air within the next 6 hours.
-Not having a headache.

I have been informed and certify that I have read and understand the botox benefits and risks agreement written in this document; and I certify and understand that I am aware, I also understand that I may need one more session to complement the acceptance of the product and the  maintenance of the result, therefore it is necessary to attend in 2 weeks for a re-evaluation and fixation of the botox in the area of ​​application.
